Please read the Parent/Student Handbook for 2024-2025 school year which can be found on the main page of our website. After reading it, make sure to download, print, and return the signed forms to the school as soon as possible. This form is per student so if you have more than one child in the school you would need to send a form for each child.

Sports Participation Medical Eligibility Form

This form must be completed by your child’s physician before he/she can play in any sport. If your child is interested in participating in any sports, please complete this form and return to school as soon as possible. The children cannot participate without this form in our possession. These forms are different from their annual physical.
