Please print and fill out one form per student. Return promptly to your child’s Homeroom teacher.

This form must be completed by your child’s physician before he/she can play in any sport.  If your child is interested in participating in any sports, please complete this form and return to school as soon as possible. The children cannot participate without this form in our possession.  These forms are different from their annual physical.

Only return this form to school if you DO NOT want your child’s picture be posted on the school’s website and social media.
